This rewards program features a card for each patient; and with cooperation during treatment, the patient will receive a stamp that will lead to fabulous rewards!

When you come in to have your braces put on, an assistant will give you a “Stamps to Smiles” card.  Bring this card to each of your visits with Dr. Cosmo to get your card stamped.  If you are collecting tokens, you can hold on to them or trade them in for stamps on your card.

You will have the opportunity to collect a maximum of three stamps at each of your regularly scheduled visits!

One Stamp: Arrival on time to your appointment
One Stamp: Good brushing and home care
One Stamp: Good elastic wear or no broken brackets or appliances

11 – 15 Stamps: $5 Gift Certificate
16 -20 Stamps: $10 Gift Certificate
21 – 25 Stamps: 2 Movie Passes
26+ Stamps: $25 Gift Certificate

Tips for maximum rewards:
Make sure your name is on the card and keep it in a safe place because if you lose it we will give you a new card, but you will lose your previous stamps!
